If I understand you correctly, you see the same problem when running fping from the command line. So the problem is not in Smokeping at all. Maybe something is rate-limiting your ICMP packets? This could be a host-level firewall (netfilter/iptables) on the local host or a device between you and the targets. Other than that, no good ideas.
