Joe, The AMA is supposed to put the forms on their web site. According to Steve Kaluf;
You can call The Competition Department at the AMA,
You can get the phone number from the website.
Amy or Steve Kaluf, will be happy to send a package to you .
If you have been to the Nats in the Last 2 years.. You will be receiving a package.
The New LSF admin may put them up on the LSF site..in due time.
