There was a bug handling stateless requests in the fix for the
CANCEL-instead-of-PRACK bug. The requests were freed twice under
certain conditions, which is what you seem to experience.

If the new fix for that (now in darcs) does not help, I'd like to see
message traces between SIPP and Freeswitch (generated with e.g,
wireshark or just output with environment TPORT_LOG set).
