Summary: BYE sent after a 200 flood is not reported to the client

Initial Comment:
A peculiar problem was triggered when I tried a call from an account at 
proxy01.sipphone.com to another account at ekiga.net.
At the caller side, there happens a flood of 200 OK retransmissions from the 
callee, each of which is duly ACKed by the caller, but still the responses keep 
coming in. After some time, the caller's NUA/NTA becomes fed up and sends a 
BYE, which promptly gets a 200 OK response. For the callee the call is 
terminated, but the caller never gets a state change or termination event from 
the stack about the BYE request sent.

Looks like nta tries to be too smart and sends a BYE by itself. Because
BYE is sent by lower layer, the caller's nua is not aware of it.

A call trace with TPORT_LOG=1 and NUA_DEBUG=7 and a
tcpdump/ethereal/wireshark trace from the caller  would be very helpful...
