Rename:
MKKDEBUG -> MKDEBUGKERNEL
MKTOOLSDEBUG -> MKDEBUGTOOLS
while keeping compatibility with the old names. Add missing documentation.
Now all debugging tunables are prefixed with MKDEBUG.
