Finally delete the ugly -O mdoc option.
This option no longer does anything since i just changed formatting
of the mdoc(7) page footer to agree with the man(7) page footer.
I had hoped to get rid of this option for more than a decade; kristaps@
only grudgingly OK'ed it as a temporary measure on Dec 4, 2011.
