Major cleanup (but there is still more to come):
* rewrite .An, .Bd, .Bk, .Bl, .Ex descriptions
* correct "parsable" to "parsed"
* and various formatting and wording tweaks
This commit includes a patch from kristaps@ explaining empty .Dd.
