Hmm that's odd... I always use a netinstall CD for my kickstarts... I use a custom isolinux.cfg file to have labels predetermined for a variety of configurations.... DB, base, web slave, varnish proxy etc depending on the role the system is to have... The kickstarts differ by activation key used (for different packages/channels and configuration channels) and network configurations...
To save space on my ESX farms I use a netinstall cd to kickstart them (dont' have access to dhcp in my environment for PXE) since the ~5-9MB netinstall for teh purpose of kickstart is much better than the space for the CD/DVD.... One thing that does ring a bell given you say the CentOS first CD works for this is... I have seen similar behaviour when attempting to use, say, a 5.0 RHEL CD to kickstart from a 5.1 repo mirror... and vice versa. If memory serves anaconda doesn't like booting from kernel 'A' and then finding kernel 'B' in the kickstart distro tree.... What base (5.x) version was the netinstall iso you were trying to use? I see you have a 5.4 tree.... If you need help building a custom iso for kickstart purposes let me know and I'll share my build scripts and isolinux behaviour.
