I had a heck of a time getting Outlook to reliably detect the X-Spam-Flag header. I had the best results detecting "X-Spam-Level: *****" (or your preferred number of stars). I finally just used procmail to send the spam to a different POP mailbox and let Outlook filter based on the account it came in on.
