By default, qmail-scanner only checks to see if the message is SPAM, but
doesn't do any of the 'normal' spamassassin tagging/Header-addition.
I assume you are using spamc/spamd. To allow spamassassin to mark up
the messages, you have to change one of the lines in the
qmail-scanner.pl file.
Change
my $spamc_options=' -c -f';
to
my $spamc_options=' -f';
