Have you tried running the milter with increased logging? Also try running the milter with the options what will disable the body modifications and subject modifications to the mail the milter does (well just for debugging to see if the problem is somewhere in that procedure). Maybe it was -m parameter to spamass-milter - check the docs to be sure. You might want to try some other milter like miltrassassin or MIMEDefang, or maybe MailScanner to call SpamAssassin.
