We are using Qmail-Scanner (and changed it to deliver mails to a global Spam box instead a per user Spam Box) to call SpamAssassin.

That all depends on how you are calling SA. If you are using procmail, as I
and many others are, you could right a simple procmail recipe that checked
for the presence of  "X-Spam-Status" and if it wasn't there, passed it to
SA.
