I use mailtools.anomy.net and clamav. I drop attachments at the server that have these extensions: ADE|ADP|BAS|BAT|CEO|CHM|CMD|COM|CPL|CRT|EXE|HLP|HTA|INF|INS|ISP|JS|JSE|LNK|MDA|MDB|MDE|MDZ|MSC|MSI|MSP|MST|PCD|PIF|REG|SCR|SCT|SHS|URL|VB|VBE|VBS|WSC|WSF|WSH

These are the same ones that ms said outlook bocks.

You can increase the score for MICROSOFT_EXECUTABLE to something over your threshold, as a temporary measure. This catches new virus types before the AV vendors even know about them. It also prevents innocent executable attachments from going through, so a quarantine / review process is a good idea if you use it long term.

Does anyone send legitimately .exe's through email anymore? It seems to
have become an unwritten law that executables are more likely than not to be
stripped. Most recent versions of Outlook won't let you open .exe/.bat/.scr
attachments.
