The rule is hitting USER_IN_WHITELIST. The entry was removed days ago. A grep was done on all cf files and no other reference  was found. Any ideas?

The white/blacklists are definietly NOT cached anywhere. Period.

However, you might be experiencing lag effects due to the AWL. Check your
headers for the list of rules hitting, not just the scores.
