It turns out that our ACS server was not passing the proper privilege
level. For some reason this didn't bother our 6500 series switches but
it did impact all other devices. Once we specified the EXEC privilege
was 15 (even though it should have been without manually setting it)
everything started working...

We are using NCM and it works flawlessly on Cisco 6500 series switches
but other Cisco switches (e.g. 3500 series, 4500 series) the results is
always "Cause of Failure: Capture failed" Anybody got any ideas?
