All the other snippits of logs etc show that your USB is working fine.
Your problem is between pppoa and pppd, I'm guessing a config thing.

Could you show us your peer file (without password!) and possibly run
the pppd syslog again with the debug flag set in your config file.

If this is a custom kernel, you did eneable ppp support?
