We could copy from http://creativecommons.org/choose/ I would drop the custom license option entirely, it just doesn't seem practical / worth it. The "Original Author" thing makes me wonder: Shouldn't there be a feature where you specifically submit a derivative work, associating it with the original item (be it on SU, too, or elsewhere)?
