OK, using algebra, (don't know)/(don't know) = 1. Since the question of how often leaks occur only comes up when a leak occurs and the answer is 1, could this then be the source of the inevitable answer "this has never happened before"?

Do not believe or repeat any statistic on sprinkler leakage you hear, because there is no way to develop one. No one has even a remote guess as to how many sprinklers are actually out there. There is also no consistent collection of data on leakage incidents. If failure rate is determined by dividing the number of leakage incidents by the number of installed sprinklers, the answer is "don't know" divided by "don't know", which is "really don't know".
