And as obvious as it sounds, I'd caution against a 12" offset since you'll
hit ceiling wire.

Many designers throughout the country have told me the same thing. If
they are installing the straight FlexHead's they offset the sprinkler
head 10" to 14" from the branch line. I have also been told by many
contractors that they only use the FlexHead elbow unit and then they do
not have to worry about the offset because the welded elbow allow them
to position the sprinkler head as close to the branch line as they want.

I have been told at least 10" from horizontally from the sprinkler to 
the outlet. I haven't heard of any particular distance above the 
ceiling as other thing usually dictate that.
