If you look at the aboveground contractors' acceptance form from 13 it
has a question as far back as I can remember asking about whether
there is a coupon recovery procedure. I believe the meaning is kind of
like for an operating room where all the tolls, sponges, bandages,
etc. are identified and counted going in and coming out. The form
wants to know if you have a method for making sure there are the same
number of coupons as there are holes. You can count them and the
number of holes, attach them at each tee, put them all together in a
box and make them part of the as-builts, you could flush each section
of pipe, you could send a camera down every line in the system--you
just need to have a system (key word) for knowing there aren't any
coupons forgotten in the pipe. Obviously in your example there was no
system, an inadequate system, or just a mistake. I've never been
comfortable around someone who says, "Don't worry, it's not loaded."
He and I are coming from a fundamentally different mindset.
