Other than the 'closed valve' scenario? I do not believe, in my 38+ years, 
I have heard of such a failure. I believe the stats bear out that the 'old' 
pipe schedule method (with standard-response sprinklers), typically 'fused' 
2 to 4 or 5 sprinklers and was more than adequate for containment or 
extinguishment. I have not read or received materials bearing out a "Pipe 
Schedule" 
failure due to the system actually being "Pipe Schedule"...
