Good points - my answer was too general.  So long as you meet the flexibility 
and/or annular space clearance requirements, the penetration can be of any 
approved configuration.  So a floor slab could be blocked out when it's poured, 
sleeved, cored (not likely but I suppose it could happen), etc.

Doesn't "9.3.4 Clearance" only specify a minimum dimensional allowance of space 
around pipes passing through structural elements?  I didn't see where it 
actually requires sleeves (ref. 2007 NFPA 13).

If this sleeve is required on the vertical run-in as the UG pipe passes through 
the floor, that's not uncommon as often in new construction the sleeve allows 
the concrete to be poured leaving the opening and not requiring core drilling 
or cutting later.  More of a preference for construction than an actual 
requirement.

If it's in the horizontal run on the UG pipe as it passes below or through 
footings or foundation walls then that's another issue.  Some of those details 
have been copied and re-copied from jobs where there was foundational loading 
on the UG pipe or the UG pipe was plastic and needed to be run in a sleeve to 
protect it from the vertical load of the foundation above.  Unless the 
structural engineer has warned the ME that there could be issues with pipes 
passing under these structural elements, typically for ductile iron pipe 
there's no need for a sleeve around the fire run-in.
