If I recall correctly, the chain of events surrounding the Omega sprinkler head
debacle started with Central Sprinkler Corporation. After reports of several
sprinklers failing to operate when the links fused, Central acknowledged the
issue and issued a quiet voluntary recall involving a handfull of sprinkler
contractors across the country. When this went public, CSCs' publicly traded
stock took a nosedive and Tyco moved in and purchased the assets and
liabilities of CSC. CPSC then issued a forced recall, which they did monitor
and enforce. My recollection is that it had a 5 or 7 year term and was thought
to involve some thirty plus million units. The rest, as they say is history,
and so was the venerable Central Sprinkler Company.

I had always assumed that voluntary didn't mean voluntary on the part of the
consumer anymore than any recall is, but voluntary on the part of Tyco, meaning
CPSC reviewed and accepted Tyco's self-initiated plan. This took CPSC our of an
enforcement loop and allowed Tyco to manage how the recall and replacement
would be administered. This allowed Tyco to spread the cost over a longer time,
and with more autonomy than would normally be allowed if CPSC was
administering. That is at least how I understood what was explained to me.
