Could you post more of the tex file?  I still don't see the
tabledef.

What happens if you replace the <%itemnotes%> field with some
hard-coded text, as a test?

What happens if you take out the close paren?

What happens if you do both of the above?

Get it rendering properly then add stuff back one piece at a
time.
