It means that the programmer is using the SQLite API incorrectly, and that there is a fundamental flow or logic issue in the code. An example might be calling sqlite3_bind_xxx() on an "active" statement (one that has been stepped, but not yet reset/finalized). Such action simply does not make sense. It is usually the result of a new-to-SQLite programmer not fully understanding the correct way to use SQLite API, but can also point to buggy flow control.
