SQLite's implementation makes things worse because the xOpen implementation is not 5 lines of code wrapping a call to open() but considerably more meaning that overriding it is a lot of work. As for the sandbox, the traditional Unix mechanism for doing that has been chroot which works well for daemons, but is too hard to use for something like the Flash player or webkit renderer.
