When were the appended tracks added? I have noticed that if I add a track in the closing few seconds of the track that is playing the appended track doesn't start automatically. Other than that I don't experience this issue.
This actually could be true: LMS would start pre-buffering the next track about 10s (or the buffer size - don't remember) before the end of a track. If you add tracks after that moment, LMS likely would miss them.
