My Last.fm account also had to be re-authorised since I upgraded to 7.3 (it may also have been as a result of the SN upgrade). You don't need to do anything with the checkbox, I'm not even sure what it's for. I think it's used to delete your Last.fm account from SqueezeNetwork (the checkbox actually has name="delete" in its HTML properties), but it could certainly do with some explanation on the page.
