This would happen if you or your LMS did trigger an unusual number of requests. We've seen this happen with some instances which would send thousands of login requests to our servers within a few minutes. I have never figured out what would be causing this, even added checks in LMS to prevent it etc. But still, it happens. It's usually good enough to shut down LMS and restart it.
