Im not sure how much help I can be as I don't have any of my receivers
at my current location, and I really don't use mysb.com (i.e. haven't
reset receivers to go directly to mysb.com). Also, you say that all
players can connect to LMS, so that would tell me that the hardware is
behaving???

My only thoughts - 
1) All my receiver problems have been due to connecting via wifi, try
ethernet at least just temporarily.
2) I always reset my receivers and controllers together. That is, if I
have to reset a receiver, I reset a controller to help through the
setup. I have neve had any luck just resetting the receiver.
a) Press and hold button on receiver until fast red flash that will
eventually go to slower red flash.
b) Power off controller. Hold '+' as you press power button until
buttons flash then release.
c) when the controller comes up it goes through the process of
connecting to the network, then finding a receiver, at which point the
receiver buttons go through the light change process.

3) Remember too that mysb.com only shows the players "connected" if they
are indeed connected to mysb.com, not if they are connected to LMS. If
things are working well you can switch back and forth - "connect to my
sb.com" and "my music" takes you back to LMS.

You mention some messages you receive, some seem to be from the players,
some from mysb.com? Maybe if you just shut down LMS for the time being
and focus only on mysb.com, to eliminate some of the
confusion/variables.
