I'm using Squid version 2.5.STABLE4 on a Debian Woody machine. Since 2.5.2 I
have the same problem when I try to reach a few number of web-sites, like
www.ite.com.br, www.ite.edu.br, and www.aduaneiras.com.br, from my Windows
98 clients and IE 6. I did not try with others browsers, since when I try to
reach through masquerading (made by the same linux server), I can get to the
websites (the DNS server is the same in all cases).

The log record in access.log is:

1069662258.674  60752 192.168.11.54 TCP_MISS/000 0 GET
http://www.ite.com.br/ usuario NONE/- -

Cache.log doesn't record anything.

Does anyone have an idea?

Seems that you are trying to use Squid as a transparent proxy,
am I right? If so, did you configure squid's httpd-accelerator mode?
If not, take a look at http://www.squid-cache.org/Doc/FAQ/FAQ-20.html#what-is-httpd-accelerator
