Looks the MRTG perl script has some bugs or issues.

Indeed your attached cache.log shows that 1.3.6.1.4.1.3495.1.5.2.1.2
is not a valid OID for the running Squid process.  Maybe the MIB file
that MRTG is using is out of date?
