The only case I have seen is people forgetting to run "squid -k rotate"
every now and then, causing disk usage to grow out of control for log
files and swap.state.

I do not know of any issues with Squid-2.5 where the cache usage grows
above the assigned size.

Would you like any config files or anything from my setup? It's definitely
growing beyond the size I set it to (assuming I have the setting done
correctly in squid.conf):

cache_dir ufs /var/spool/squid 5000 60 256

Also, is there a way I can remove some of the cache without killing all of
it?
