No there is only one squid instance and it's the problem.

Some constraints work against two instances (I need to keep same
config on all servers, and only one have a proxy provider).

Maybe I will try to hide these log with syslog-ng.

1 & 3 being the same Squid instance?

If so I'd recommend using two Squid instances with different
unique_hostname.

If it's already two Squid instances on the same server then just set
unique_hostname differently to allow them to tell each other apart..
