Amos this is one of the weirdest things I have ever seen!!
the more in depth logs shows a 304 request and response while the access log shows 200.
So I noticed that there is a Pragma header present like this:

So actually what can cause this kind of a problem??
the 304 is what I would have expected but I see that Pragma effect some requests.
Can we disable Pragma Headers without modifying the request using ICAP?

The above picture should be valid for a very long time.
but still it wont be "cached" by squid.
it shows a tcp_miss but with 304 in some cases.

Which should be cachable for at-least very very long time on all caches.

Should be cacheable for up to 365 days (3153600 seconds) from its
creation/last-modified timestamp ... which is set to a time more than
365 days ago now.

The 304 is expected, but should be incrementing max-age value to permit
further caching given todays date.
