I got it.  There's a Gentoo patch for Berkeley DB 4.0.x support I
applied (the server is running Slackware), I upgraded my 2.2.x with
4.0.14, recompiled SquidGuard against it, which required a bit of
hacking... I think because of my old version of GCC on there.

With this db version, squidguard was able to build the B trees properly,
and ten properly use them.  It launched in under 30 seconds after taking
about a minute to pre-build the B trees.
