The default squid.conf file contains most if not all of the options,
and is very well documented. I ran squid for 3 years before I
created a stripped-down configuration file that did not include all
of the documentation; it was just too easy to make a mistake.

(Is 'hierarchy_stop' legal?  All I've ever seen is 
'hierarchy_stoplist'.)
