Are you using squidGuard for this or just squid? If just squid I think it has to do with you needing to echo the syntax, or you need to do it like .playboy.com instead of playboy.com

They fired up their browser and went to www.playboy.com. Well, they made it through. I have a block on playboy.com in the 'porn' list. In theory, www.playboy.com should be blocked also. I have 'atunnel.com' in a custom block list. both atunnel.com and www.atunnel.com get blocked, the way I would expect. Other domains in the list block both the domain, plus the www part.
