I had many of the same problems with the logins and 1.2.9. I started
removing plugins. I am now down to:

$plugins[0] = 'calendar';
$plugins[1] = 'squirrelspell';
$plugins[2] = 'delete_move_next';
$plugins[3] = 'smallcal';

I HAD to remove the file_manager plugin, otherwise I could not
login...
