Something is being appended to the subscription request. The
mail/Foomailbox[]= should *not* be there. Can you create, subscribe,
unsubscribe, and delete folders from inside SM? Just not folders
created by Outlook?

Can I suggest checking the IMAP subscription file, usually a .<name>
file in the users home directory.  I cannot remember UW-IMAP's name
for it.  Could be something like .imapsub or something silly like
that.  Do you see those (rather) curious paths in that file too?
