You can set the default folder prefix in the configuration file.  Run
the config/conf.pl script, and go to the "Folders" option.  In the
"Default Folder Prefix", but mail/ and you should be set to go.

As for the weird boxes... not sure, but UW-IMAP stores it's
subscription file in .mailboxlist, you can edit that with an editor,
like vim, or pico.
