If you have installed qmail deb package, you have /usr/sbin/sendmail. If
you have installed qmail from source, you have /var/qmail/bin/sendmail and
INSTALL says (step 15), that you should link /var/qmail/bin/sendmail to
/usr/sbin/sendmail.

It is qmail's wrapper for sendmail.

I have send my previous email with SquirrelMail using smtp. Debian woody,
qmail 1.0.3-24, SquirrelMail 1.5.0cvs. No error. I think I will get same
thing with 1.4.2 too. One of my older SM setups was using SMTP and nobody
complained about errors. Have you changed something in vanilla
SquirrelMail or patched qmail? Or maybe you are using some complex SMTP
setup in SM. TLS? SMTP auth? Pop-before-smtp? dpkg -l php4
