I don't know the particulars of Cpanel myself, but if you can log into
SquirrelMail using the normal login screen, just use the address for the
login screen as a hint and cut and paste the two together:

URL:
http://my.domain.com/squirrelmail/src/login.php


Example from SquirrelMail wiki:
http://your.host.com/mail/src/redirect.php


What your actual form should look like:
action="http://my.domain.com/squirrelmail/src/redirect.php";
