why are you changing the source, when configuration provides options for non-anonymous
binding?

try using ldap v3 binding. Last option in ldap address book config. php defaults to v2
bind, some ldap servers does not allow ldap v2 binding.
do you have ldap support in php?
ldap address book is used only when you search for some address. listing of all
address book entries is not enabled.
