The users of Squirrel mail don't know to flush their deleted items folder. Is there a way to have the mail server automatically flush the deleted items folder by date after 3 months? So when a deleted item becomes 3 months old the server would delete it?

courier imap has options that can clean folders. cyrus has utility that does same thing too
I too am looking for such a plugin, and I tried the plugin mentioned above but it will remove all email when it purges the trash. So even mail deleted 5 minutes before it runs will be gone. It would be nice if the Auto-prune Sent -- Version 1.1 plugin could be modified to also do the same for trash. Since this plugin will only remove the items older than the day you set, instead of just removing everything.
The idea is that users don't generally do anything with their trash, unless they want to undelete something they just deleted a minute ago (or five as in your case). Email deleted that recently will never be deleted by that plugin unless you happen to delete it, log off at 23:59 and log in again at 0:01 *AND* it happens to be X number of days since your trash was last deleted (per the config). You are overstating your case.
That said, I hear the request you are making and it is a known feature request. Keep your eyes out.
Also, note that the empty_trash plugin had a small bug that has been fixed as of the new version that has just been posted to the web site.
