Secondary problem:
When I do use Addresses button I do not get any checkboxes. Just the To, CC, and BCC.
lettering. Is this normal behaviour?
I am using SM 1.4.5 with OpenBSD 3.7 (default sendmail and apache).
I can't help with your first problem, but the second should be easy. Under "Options", then "Display Preferences", there is a drop down box labeled Addressbook Display Format.
Change that from _javascript_ to HTML, and your Addressbook will have the checkboxes.
