So then we're down to Hotmail and their spam filters. It may be that they trigger on some SquirrelMail specific headers in the mail. SquirrelMail has configure options for not using the SquirrelMail specific headers, but off the top of my head I can't remember which settings they are and in which SquirrelMail versions.
