However at over 3kB, your file format appears wrong for a 16 x 16
image in 16 colours.
    

This is the same favicon I have loading from the root of Apache and it
loads fine there for whatever reason, just not from the SM directory.
I simply did:

cp /var/www/html/images/favicon.ico /usr/share/squirrelmail
