OK I got it.

by looking in my system logs, I saw the following:

Dec 23 22:41:36 <myhost> sshd2[4751]: connection from "2.2.2.2"
Dec 23 22:41:37 <myhost> sshd2[4753]: DNS lookup failed for "2.2.2.2".
Dec 23 22:41:41 <myhost> sshd2[4753]: Connection from 2.2.2.2 denied. 
Authentication as user <username> was attempted.
Dec 23 22:41:41 <myhost> sshd2[4753]: Remote host disconnected: No 
further authentication methods available.
Dec 23 22:41:41 <myhost> sshd2[4753]: User authentication failed: 'No 
further authentication methods available.'

This appears to be caused by a reverse lookup problem which I fixed 
by running routed on my firewall.

This might be a good candidate for the FAQ.
