I've already responded to Mr. Smith on this topic, but failed to post it here.

SecureCRT from VanDyke software meets most of your needs below.  It's a little
pricy for licencing, but is a very solid application.

Not sure about command line, but it is a gui for Windows.

telnet, rlogin, ssh1 (RSA, Password, TIS), ssh2 (Password, PublicKey).

Don't know.

There is an additional program (SecureFX) which supports ftp via ssh2.

Yes, both ssh1 RSA keys and ssh2 PublicKey styles.

Don't think so, although it does support sftp via ssh2 (see above) if you also
get the SecureFX package.
